Adult Volunteer - Report of Medical History

Though a physical is not required, the Medical History form must be completed. This document will be filed in the adult service record.

• The Report of Medical History must be updated:
  1. annually upon re-enrollment;
  2. upon any change to physical or mental health history; and
  3.  within 30 days of reporting to any NSCC or NLCC training.
